Select the Clearance in the Major form for your catalog year. Fill it out completely, and take it to your advisor one year before you wish to graduate. These forms have both a front and a back (see the bottom of the excel spreadsheet to toggle). Detailed instructions are given on the form. After obtaining the appropriate signatures, submit the form with your application for graduation.
