Group Forming Survey


You must complete this by noon on Tuesday January 24, and you must attend class on Wednesday January 25.

In order to be certain that I receive your e-mail, you should use ecst webmail to send your e-mail to me.  E-mail from ISPs does not always get delivered on time!

If you forget to complete this survey and/or you do not attend class on Wednesday January 25, there is a very good chance you will be in a poorly selected group.


What you have to do:

Send an email to tyson@ecst.csuchico.edu with the subject "330 survey answers" and a single line as the body of the message with the following format:

last name, first name, primary e-mail address, answer to question 1, answer to question 2, ... , answer to last question.  

For example, my answers are:

Henry, Tyson, tyson@ecst.csuchico.edu, really enjoy, hate, definitely, absolutely not, probably not, definitely, probably not, maybe, collaborate a lot, soon enough, marathon, very social, maybe, consensus building leader, 6, 3.75, Idealist

I will be pasting your answers into a spreadsheet so you must use commas and the format must be exact.  Double check that you have 17 answers and they are in the correct order.

I will share your answers to questions 1-15 with the rest of the class during the group formation process.  I will not share your answers to questions 16 and 17.  

If for any reason you do not want to answer one or more of the questions (e.g. you feel the question violates your privacy) you may leave your answer blank (in other words, leave a blank between the commas).  For example, in the following I've left the answer to question #2 blank:

The Questions

When a question is followed by a set of italic phrases, answer with one of those phrases.

The Next Step

I will take the first 15 answers for all students and put them in a spreadsheet.  I will pass out this spreadsheet in class during the second lecture.

During the second lecture each student will get a chance to say something about themselves.

All students will then submit a confidential "work with" preference for all other students in the class (see Group member preferences  exercise).

I will use the survey answers and the work with preferences to create groups.
